TYPES OF URINARY LEAKAGE
1. Urge Incontinence – This is leakage caused by overactive bladder muscle or nerves. This leads to urinary urgency followed by uncontrollable leakage of urine. Many patients may also have urine leakage without prior urgency.
2. Stress Incontinence – This is due to leakage from a weak sphincter. Typical symptoms include leakage with coughing, laughing, running, standing, or jumping.
3. Overflow Incontinence – Overflow incontinence is from retention of urine. The bladder is so full urine is leaking out constantly. This may be due to bladder nerve dysfunction, muscle dysfunction, or constipation.
GENERAL TREATMENT OVERVIEW
LIFESTYLE CHANGES
Lifestyle changes include a plant based diet, water (avoid coffee, tea, and soda), stress reduction (yoga and meditation), sleep improvement (treat sleep apnea and monitor REM sleep), exercise modification and weight loss.
MEDICINES
Medications are sometimes an option for urge incontinence. They include oxybutynin, vesicare, and myrbetriq.
MINOR PROCEDURES
Most procedures are done under sedation in our surgery center as an outpatient.
ADVANCED INTERSTIM FOR URGE INCONTINENCE
Advanced Nervestim is a proprietary technique utilizing Medtronic’s Sacral Nerve Stimulation (or Interstim) technology that provides the best outcomes with minimal side effects for urgency and urge incontinence. This is similar to implanting a small pacemaker or nerve stimulator for the bladder. We utilize Advanced technology with 3D fluoroscopic mapping to precisely fix the root cause of overactive bladder. This targeted millimeter accuracy improves outcomes and minimizes side effects. Urologists from around the world come to Advanced to learn from our talented team.
ADVANCED BOTOX FOR URGE INCONTINENCE
Advanced Botox is used for urge incontinence. Botox blocks nerve signals from reaching the bladder muscle and as a result it reduces urgency and urge incontinence. Botox is injected through an endoscope in the bladder every 6 months in any of our 9 Atlanta surgery centers. There are no cuts or incisions and the procedure typically takes about 10 minutes.
FEMALE MIDURETHRAL SLINGS FOR STRESS INCONTINENCE
Slings are used to treat stress incontinence. Slings can be autologous (your own body tissue) or mesh (synthetic). Slings provide instant relief from stress incontinence. They are generally performed through a tiny vaginal incision as an outpatient in our amazing state of the art surgery centers. You are sent home the same day after a short 30 minute procedure. Your leakage is immediately fixed after the procedure.
MALE SLINGS FOR STRESS INCONTINENCE
Male Slings are used to treat stress incontinence in men. Men generally only develop stress incontinence after surgery on the prostate or from radiation. Slings provide instant relief from stress incontinence. They are generally performed through a small perineal incision as an outpatient in our amazing state of the art surgery centers. You are sent home the same day after a short 45 minute procedure. Generally a catheter is required for 3 to 5 days post procedure.
ARTIFICIAL URINARY SPHINCTER
Artificial sphincters are mechanical devices placed around the urethra to stop stress incontinence. They act similar to a blood pressure cuff around the urethra. Normally they stay closed (as your sphincter would) and when you release the valve the cuff relaxes and urine can flow out of the urethra. Artificial sphincters are a great way to fix male stress incontinence. They are very rarely placed in females since female midurethral slings work so well.

OVERFLOW INCONTINENCE / URINARY RETENTION
Urinary retention generally results from underactive bladder nerves or muscle. The short term solution is intermittent catheterization of the bladder. Patients are generally sent to rule out neurologic causes of urinary retention (inability to urinate or fully empty) to ensure no significant diseases are undiagnosed such as multiple sclerosis. The best long term solution is a nerve stimulator with Advanced Nervestim to stimulate the bladder nerves and bladder muscle to contract.
