Surgeon Fees
Surgeon fees include those of the surgeon, physician assistants, nurse practitioners, and certified surgical first assists.
Anesthesia Fees
Anesthesia fees cover the cost of the anesthesia provider, necessary medicines and equipment to keep you safe, asleep, and pain free.
Facility Fees
Facility costs are those of nursing staff, operating room staff, and equipment necessary to provide high quality care. Your cost includes initial visit, surgery, and one follow up visit for one problem.
Ancillary Testing
Ancillary testing for the same problem is covered – this includes ultrasound and urodynamics. Lab and pathology are not covered.
